There are several mechanisms by which SB may prevent and relieve diarrhoea. Disruption of the intestinal microflora occurs in many pathological situations such as infectious diarrhoea and diarrhoea and colitis linked with antibiotic treatment.

Antibiotic associated diarrhoea (AAD) develops through the loss of normal bacterial intestinal flora. AAD is used to describe any unexplained diarrhoea associated with antibiotic use, including infection caused by Clostridium difficile. A rise in antibiotic use has resulted in an increase in ADD.
